1. Driver’s License and Social Security Cards for Taxpayer, Spouse and Dependents
2. Dates of birth for dependents
3. Death Certificate if taxpayer, spouse, or dependent died during the year
4. W-2s We need all of these forms.
5. Pension or Retirement Income
6. If you have copies, last year’s Federal and State Returns
7. Self-Employed Business Income and Expenses
8. Child Care Expenses and Provider Information (Name, Address, and Employee Identification Number for provider)
